Social Services

 

 

The Social Services Department at Kell West Regional Hospital is staffed with social workers who interact closely with nurse managers, utilization review coordinators and nurses to enable proactive discharge planning, patient/family advocacy, cost effective quality care and access to resources and entitlement programs.

 

A representative is available to assist patients and their families before, during and after their hospitalization.  Services include referrals to necessary sources, such as home health and hospice care, nursing home placement, substance abuse counseling, medical equipment needs, protective services/counseling, as well as advising you on services provided by other community services or governmental agencies.

 

We are concerned about your health, even after you leave our hospital.  If your recovery requires special care after your stay, social services assistance is available to help plan for your discharge needs.

 

Discharge planning for professional services or equipment is initiated by your physician.
